How does health insurance work in the US?I am a non-US citizen and need this information to do a case.
Specifically:
1) Is health insurance compulsory for everyone?
2) What happens if someone cannot afford it?
3) In the event that a medical procedure needs to be done, does health insurance cover all the bills? Does the patient need to pay anything extra?
4) Does the patient have any say over what kind of procedure he can take? Say if 2 treatments are available for his condition, can the patient choose the more expensive treatment? And if so, is it covered by the insurance?

Does a health insurance contract state that it will only cover the "normal" rates for a procedure? For eg. if there are 2 possible treatments for a disease, 1 of which is more expensive but more effective than the other, will the patient only be covered by the LESS expensive one?
Or is it a case in which the patient can opt for the more expensive one and "top-up" the difference?

The baby has to "pass" their first few medical checkups to make sure they have no expensive medical conditions before the insurance company will accept them. Most of the insurers I'm familiar with won't cover a newborn until they are anywhere from 6-10 weeks old. (The only exception to this would be if you already have family coverage and just want to add the newborn to your existing policy – if that's the case, check with your agent/insurance company to find out how to do that.)
You can gather information on insurance companies ahead of time, but won't be able to purchase any coverage until after the child is here and after they are determined to be "healthy." Sorry.
Otherwise, you can inquire to see if you meet the financial requirements for Medicaid or other public insurance programs for children in your state.
P.S. Either you or your husband could seek a job that comes with group health coverage…the baby would be able to be added to a group policy also. Just one final option to consider.

The problem is, people in excellent health and under 40 can get private health insurance at about that premium, or slightly less – so the people you're going to get to join are going to have pre-existing conditions and otherwise skew the general health of the group. Plus, you're talking about them adding families, right? You CAN'T insure a family for $300 a month, you just can't. CHEAP family insurane is going to run closer to $600 a month, IF everyone is healthy. Which, I'm assuming, you guys aren't.
Lastly, 1000 people isn't a very large pool; AND when you're talking about international coverages, you're talking multiple carriers, with wildly different governmental regulations, and it's REALLY going to be more hassle than it's worth from ANY agent or carrier's point of view.

One of them might have to find a job that has health insurance. Many people have been forced to do that, giving up being able to retire or own a business because they need the medical insurance.
They can try some high deductible plans. If you choose one with a $5,000 deductible it works if you get cancer or need a heart transplant but you are on your own for office calls so it is much cheaper.
Check with your state insurance commission they may have a high risk pool, it won't be cheap but they can get coverage.
